Egg Donor Questionnaire

We are currently recruiting ovum donors from the greater eastern Washington, northern Idaho, and western Montana areas for the IVF program in Spokane, WA. Current compensation for ovum retrieval through this program is $4000. For information about donating eggs, please call our donor information line at 1-509-232-0134. If you are still eligible for this great program after reviewing the criteria provided on the "info" line, then download the Ovum Donor Personal History form, fill out the questionnaire, and mail, or bring it in for review.
