The completed original registration forms found in this section must be submitted and processed by all clients who wish to purchase long donor profiles and/or commercial donor sperm. All clients wishing to order sperm specimens must be 18 years of age or older and must include proof of age with client paperwork. Legible copies of current drivers license, birth certificate, or other proof of age must be submitted with the forms below. The dewars used to ship specimens may required a $500 refundable deposit prior to shipments within the USA (dewar/equipment deposit is required from all international clients). We are not able to accept credit or debit cards for equipment deposits. You will need Adobe Acrobat Reader in order to download these forms. We are happy to accept a faxed copy of registration forms to begin the client registration, and ask that you send the original by mail. You must complete the registration process before orders for donor sperm or donor profiles can be accepted. Please be sure to review your paperwork for completeness prior to mailing.
We will acknowledge receipt of your paperwork by email. Please provide us with your email address and either check your junk folder or add the following email address to your known address book to make sure you receive our emails. Your email address will be used only for communication, we don't send advertising or provide your email to anyone else.
Please file the appropriate paperwork for working with, and/or without a physician. The appropriate paperwork is dictated by who will be performing the inseminations not by where specimens will be shipped.
If you want the flexibility of working with or without a physician, then you will need to have both forms on file with us.
All client registration forms are held current, unless we've received no orders in a 12 month period of time. After that time, you may be asked to submit new registration paperwork
